Daily Health Screening

This health screening form must be filled out before class on the day of preschool.

It must be completed on the day of  class, no later than 30 minutes prior to class start-time (AM classes-by 8:30; PM classes-by 12:00).  Note to Nannies and alternate caregivers:  Please answer only on behalf of yourself and not the child or family in your care!                           Bookmark this page for your convenience.

Your child will not be allowed to enter preschool if you have not completed the form on the day of class before arrival at preschool.  There will be no exceptions.

    In the last 48 hours, has your child or any of their immediate family, caregivers, nannies, or participants in their social bubble:

    1. Had a fever or chills, cough, sore throat, runny nose, shortness of breath,diarrhea, or felt unwell?

    2. Been in close contact with someone has had sickness symptoms such as fever or chills, cough, sore throat, runny nose, shortness of breath,diarrhea, or felt unwell?

    3. Been in close contact with someone who is sick?

    4. Been in close contact with someone who has Covid-19 symptoms?

    If you have answered YES above, please do not come to the preschool. If the answer is NO, we’ll see you in class!

    Please DO submit the form in either case so that we can notify other families if necessary. Contact your health care provider for further guidance.
